EquiFlow Racine is committed to replacing all lead service lines in the City of Racine. To have your lead service line replaced when work reaches your block, you must complete and submit a Right of Entry (ROE) Authorization Form. The ROE form grants permission to the City of Racine, EquiFlow Racine, and its contractors to access your property and replace the portion of the service line you own. Completing this form ensures that both the public and private sections of the line can be replaced at the same time. Only the property owner or an authorized representative may sign the ROE form. If someone other than the property owner (such as a tenant, family member, or property manager) is signing on the owner’s behalf, they must provide a notarized letter or legal documentation confirming their authority to do so.
How to Sign Your ROE Form
You can complete the ROE form online or by mail.
Online: Complete and sign your ROE form by using the form below. You will receive a confirmation email with a PDF copy of your signed form.
Paper Copy: To request a paper form, contact the EquiFlow Lead-Free Program at inforacine@yourcip.com or call (262) 293-6936. Completed forms can be dropped off at City Hall (730 Washington Ave, Room 103) or placed in the red drop box on the west side of the building.

What Happens Next?
Once your ROE form is submitted, an EquiFlow team member will contact you to schedule your lead service line replacement when construction is set to begin in your area. For any questions or assistance, contact the EquiFlow Racine team.

Click this option if you own the property but rent or lease it to someone else. You will be asked to provide the tenant’s contact information so the EquiFlow team can coordinate access and communicate important information about the replacement.

Important Note: Only the property owner may complete the ROE Agreement. If you are a tenant, please contact your landlord or property owner and ask them to complete the appropriate ROE form.
