EquiFlow Racine is committed to replacing all lead service lines in the City of Racine. To have your lead service line replaced when work reaches your block, you must complete and submit a Right of Entry (ROE) Authorization Form. The ROE form grants permission to the City of Racine, EquiFlow Racine, and its contractors to access your property and replace the portion of the service line you own. Completing this form ensures that both the public and private sections of the line can be replaced at the same time. Only the property owner or an authorized representative may sign the ROE form. If someone other than the property owner (such as a tenant, family member, or property manager) is signing on the owner’s behalf, they must provide a notarized letter or legal documentation confirming their authority to do so.

Online: Complete and sign your ROE form by using the form below. You will receive a confirmation email with a PDF copy of your signed form.

What Happens Next?
